Why am I still exhausted when my bloodwork is normal?

Usually because the testing was narrower than the problem. A standard panel is a screen for disease, not a map of how you are functioning. Fatigue that persists through normal results generally means either the right things have not been measured yet, or they have been measured and read against the wrong range.

Your symptoms are real even when a lab reads inside its reference range. I want to say that plainly, because most people who ask me this have already been told the opposite.

There are a few common reasons for the gap. Thyroid function is often assessed by a single marker when the full picture needs several. Iron can be adequate on one measure and depleted on another. Hormones shift across a cycle, so a single draw on a single day can miss what is actually happening. And cortisol, which shapes energy more than almost anything, often is not measured at all.

None of that means something sinister is hiding. It usually means the question asked of the blood was "is she ill?" and the question you need answered is "why does she feel like this?" Those require different tests.

So we start again, properly: a fuller panel, read against optimal rather than merely normal, and interpreted next to your history rather than on its own.
