How long before I know if hormone therapy is working?
Give it 90 to 120 days before you judge it. Some things shift sooner, but three to four months is the window where we can honestly assess how you feel and how your body has responded. Finding the right dose and the right route takes time, and that fine-tuning is normal rather than a sign something is wrong.
This is the expectation I most want to set before we start, because the first few weeks are the ones people draw conclusions from and they are the least informative.
Hormone therapy is not a switch. Your body adjusts, and the way it adjusts tells me what to change. That is why we re-check labs, re-ask about symptoms, and adjust — sometimes more than once. Needing an adjustment is not a failure of the plan. It IS the plan.
The form matters as much as the amount. Cream, patch, pellet, oral — each is absorbed and metabolized differently, and the one that suits you may not be the one we start with. Changing route is a normal part of getting it right.
So the honest answer to "is this working" at three weeks is usually: too early to say, keep going, tell me what you are noticing. At three to four months we will both know a great deal more.
